כתוצאה מהתאונה שגרם השד שרודף אחר בני משפחת ווינצ`סטר ג`ון נפצע, סאמי נפצע קל ואילו דין מפרפר בין חיים למוות. בזמן שסאם מאשים את אביו בתאונה, נשמתו של דין משוטטת כרואה ואינה נראית בבית החולים.

In the closed captioning for this episode, there are a number of instances where a "Dr. Bender" is being paged, referencing the previous episode from the first season, "The Benders." In addition to the new fiery opening sequence, the "A" in "Supernatural" shown during the title sequence is a pentagram, which has magical associations, and when inverted, is a demonic symbol. At the beginning of the episode, "Bad Moon Rising" is playing in the background. This song was also playing in the final scene of the season finale "Devil's Trap." When the little girl goes into Code Blue, the announcement is made on the loudspeaker, "Paging Dr. Kripke." Kripke is the last name of the creator of Supernatural.

(his voice tails off) Bobby: Okay. You got it. (edit) Tessa: Well like you said there's always a choice. I can't make you come with me, but you're not getting back in your body and that's just facts. So yes, you can stay, you'll stay here for years disembodied, scared, and over the decades it'll probably drive you mad. Maybe you'll even get violent. Dean: What are you saying? Tessa: Dean, how do you think angry spirits are born? They can't let go, they can't move on, and you're about to become one. The very thing you hunt. Sam: (with ouija board) Dean? Dean, are you here? Dean: (as spirit) God, I feel like I'm at a slumber party. Tessa: It's time to put the past behind you... Dean: ...and go where? Tessa: Sorry, Can't give away the big punchline... John: Here. Give them my insurance. (hands insurance card to Sam\) Sam: "Elroy McGillicuddy?" John: And his two loving sons. Tessa: What’s happening to me? Am I dead? Dean: That sorta depends. John: You know when ... when you were a kid, I'd come home from a hunt, and after what I'd seen I'd be... I'd be racked. And you, you'd come up to me and you... you put your hand on my shoulder, you'd look me in the eye and... you'd say "it's okay, Dad". Dean... I'm sorry. Dean: Why ? John: You shouldn't have had to say that to me. I should have been saying that to you. You know I put... I put too much on your shoulders, I made you grow up too fast. You took care of Sammy, you took care of me. You did that, and you didn't complain, not once. I just wanted you to know that I'm so proud of you. John: Where's the Colt? Sam: Your son is dying, and you're worried about the Colt? (edit) Sam: So what, are we supposed to do, just sit here with our thumbs up our ass? Dean: Come on, Sam. Go find some hoodoo priest to lay some mojo on me. I'll be fine. (minutes later) Sam: Look, since the doctors won't do anything, then we'll have to. That's all. I don't know, I'll find some hoodoo priest and lay some mojo on him. Dean: Screw you, doc, I'm waking up! Dean: I think I'll pass on the 72 virgins, thanks. I'm not that into prude chicks anyway. Tessa: (deadpan) That's funny. You're very cute. Dean: Have you heard of an out of body experience? Tessa: What are you, some new agey guy? Dean: You see me messing with crystals or listening to Yanni?

In the hospital ICU area, Dean's spirit gets out of Dean's body, walks out of the room and goes to the corridor. In the long shot, Dean is at the end of the corridor and you can see a banner saying "Obstetrics" really close to the right-top corner of the screen. If there was an entry in John's journal about Reapers, the boys should have found it in "Faith" and already have known about it. Trivia: The date on the monitor when the doctors are resuscitating Dean is August 12th, 2006. When Dean is comatose, he is connected to several machines. However, when he wakes up, the doctors seem to have taken them all off. Even while John was in the hospital for observation he was on a heart monitor, but Dean doesn't even have this. The monitor for Dean should still be in place, even hours after he woke up. When Dean first meets Tessa on the steps, a man in a striped robe is being helped down the steps by a nurse and they head off to the left out of the shot. A minute later the same guy is seen walking back into the shot heading to the right this time walking without any help. When the doctors are doing CPR on John they are actually pressing on the right side and near the bottom of the ribs, But it should be just about over the heart. When doctors are trying to revive John, near the end the screen says, "Sensor off patient, check connection at patient. " When the doctors are shocking Dean's heart, the right paddle of the defibrillator is in the wrong spot. It's meant to be further to the right. At the beginning the demon truck driver grabbed the handle and the side mirror, ripping the door off and taking the mirror with it.
