What is the risk of doing nothing or using small doses? No known risks at 150 ng/ml as hypercalcuria or hypercalcemia has never been described at that level. In fact, the highest known natural 25(OH)D level, from sun exposure, in a farmer in Puerto Rico taking no supplements, was 170 ng/ml. Autism is a progressive inflammatory disease of the brain. Vitamin D will quell that inflammation. If you do not add aggressive doses of vitamin D, the inflammation will continue. If you exceed 25(OH)D levels of 150 ng/ml, simply obtain urine calcium (or spot urine calcium/creatinine ratios), 25(OH)D, and serum calcium often. You will seldom find abnormalities, if you do, repeat the test. If it persists, slightly lower the dose. Slightly or even moderately abnormal urine or serum calcium takes years to cause any calcification damage. Even then, it is completely reversed with sun avoidance and discontinuation of vitamin D. If you do not raise 25(OH)D levels increasingly higher, you risk missing the full treatment effect in autism and further brain damage will occur. Remember, the vitamin D treatment effect depends on both dose and time. It is easy to mistake dose effects with time effects. However, you can safely get an idea if D will have a treatment effect by giving the children 50,000 IU per day for 10 days. Totally safe, if you do not believe me, do it on yourself and check your own 25(OH)D 2 weeks after you finish the course. You will be surprised at little your own 25(OH)D level changes. Children with rickets are routinely given such doses and surely autism is as serious as rickets. The dose response to vitamin D is not linear. 1,000 IU will raise levels from 15 to 30 fairly routinely in children. But 1,000 IU will seldom raise levels from 70-85. This time of year you must keep sun exposure in mind. In the spring, 25(OH)D levels go up anyway, so the increase you are seeing is not entirely due to your treatment.
