Asking about the price range of gyms in the USA is like asking the price range of apartments for rent - All kind and all prices. I know that the gym near my home in queens asks for around 500 dollars per year. I bet that in Manhattan prices will be alot higher. You can find gyms anywhere and they are usually very big factories, not like your neighborhood gym in kfar saba... About the doctor issue- I think its like in Israel. They need it for the insurance. If you need a doctor and you dont have your health insurance to cover the expanses, expect to pay 100$ for the visit. Hope I helped
