קיבלתי למלא הטופס הנ"ל עבור בן-זוגי. ויש לי כמה שאלות: 1) מבקשים שם כתובת בארה"ב - אין לנו עדיין כזו, ולא תהיה עד שנגיע לשם ונחפש דירה. מה לרשום? 2) מה לכתוב בשאלה "Do u plan to travel" - האם הכוונה מתישהו במשך תקופת השהות בארה"ב, או לתוכנית נסיעה ספציפית ידועה? 3) מתי אמורים לקבל את הטופס I-94, שאני צריכה לרשום את המספר Admission מתוכו בטופס הנוכחי? תודה!!!

DS-2019

Prospective international scholars at US Universities need the form DS-2019 in order to obtain the J-1 visa from a U.S. embassy/consulate abroad and to obtain J-1 immigration status at the port of entry to the U.S. If you don't have an address in the US, you may use the University's address as a temporary location. I am not clear about your other question? did you mean did you make travel arrangements? Sometimes they want to know if you will be stopping somewhere before you arrive to the US. You will get the I-94 when you enter the US at the port of entry. They ask you this question because the DS-2019 is also used by International scholars already in the U.S. and might need the form to transfer sponsorship of their J-1 program, to change their status to J-1 from another nonimmigrant status, or to extend their stay. so basically u are saying that this form is to be filled with my own details (as oppose to only my husband's), and will be used for me getting the J1, so I will not need another form? and about the Admission-number (from I-94) - u mean that I should leave this line blank? thanks, Matilda.
