Tax exemption certification for US Citizen

Shalom-

I recently made a sale of Israeli stock, and to avoid being taxed income tax from the Israeli government, the brokerage requires me to present a certificate of exemption from the Israeli Tax Authority. I have searched everywhere, and found form A/114, but there are no further instructions on where to send it for processing, and the subsequent steps required to receive the certificate after processing.


Can anyone help me determine if this is the correct form I need to fill out, and if so, where I would mail it?
